Community Health

Chronic diseases such as heart disease, cancer and diabetes are responsible for 7 out of 10 deaths among Americans each year. Benjamin Franklin once said, “An ounce of prevention is worth a pound of cure,” and in fact, the right preventative care can help you stay healthy, delay the onset of disease, or keep a disease from becoming worse.

Community Health is here to provide preventative, educational, and outreach services aimed at preventing these types of chronic diseases.  We empower and encourage our patients to take control of their own health and wellness through a wide range of services, including the following:

Our team consists of a Community Health Manager, Community Health Nurses, a Maternal/Child Health Nurse, Registered Dietitians, Tobacco Prevention and Cessation Coordinator, WIC staff, Certified Diabetes Educators, Diabetes Prevention staff, Transportation Coordinator, Transportation and Program Aides, and an Administrative Assistant.
